The Scarless Breast Augmentation is now even better with Keller Funnel!





The Keller Funnel ™ has truly revolutionized the placement of silicone breast implants.

As a surgeon certified in its use, Dr. Diehl has been using this method exclusively to place his silicone breast implants from the underarm when performing his scarless breast augmentation.

The funnel works like piping bag which squeezes the silicone breast implant out and into the patient.  Its special attribute is its super-strong material which does not tear, and the patented inner coating which slides the implant out with ease.  It may sound simple, but consider that silicone breast implants have been in use around the world since 1962, and took until 2009 to develop this technology.

The funnel is a critical tool to use for implant placement because it squeezes the implant out of its narrow end and into the patient with a much smaller incision than the traditional method of pushing the implant into the patient manually.

What are the most common plastic surgical procedures?





Statistics show that cosmetic surgeries are on the rise;  the American Society of Plastic Surgeons reports that there were 13.8 million cosmetic procedures performed in the United States in 2011, an increase of 5% over 2010.

Breast augmentation has been the top cosmetic surgical procedure since 2006, at 307 thousand (up 4%).
Rhinoplasty is number two, at 244 thousand.
Liposuction was up 1% at 205 thousand.
Eyelid lift - 196 thousand.
Facelift - 116 thousand.
Non-surgical procedures (up 6%):
Botox is still the most common non-surgical procedure, 5.7 million procedures, up 5%.
Then fillers such as Restylane and Juvederm, 1.9 million.

Sunblock; is it only for summer?


Well, I treat many patients with sun damaged skin. I treat their skin with light chemical peels which removes the top layer of damaged skin exposing the new.  But unfortunately though, sometimes I have to treat them for skin cancer when the damage is too great.  I always tell my patients that it is always” the season” for sunblock and not just summer!  Sun damage is cumulative which means the damage that you have incurred since childhood will count.

Nose Surgery/Rhinoplasty



Rhinoplasty or Nose Job, as it is commonly referred, is a cosmetic procedure to reshape the nose to meet the patients' needs.  Usually the bridge is reduced and narrowed, and often that is all that is needed.  The lower portion of the nose, the tip area, can also be tightened if it is too round, shortened or lengthened if needed.  It is often elevated or rotated up a little (more often for women than men).  Rhinoplasty or Nose Job can reshape and improve appearance of the nose but it can also be used to alleviate breathing problems. 
Rhinoplasty surgery is the area of plastic surgery that is most technique-dependent. What does a Rhinoplasty/ Nose Surgery procedure involve?

Rhinoplasty procedure generally involves removing the bump and narrowing the bridge of the nose.  The tip of the nose can be reshaped by sculpting the cartilage. A splint is then applied to the bridge of the nose to help maintain its shape.

What is the Recovery like after Rhinoplasty/ Nose Surgery?

Recovery from Nose Surgery is rather mild.  A splint is usually worn over the nose for comfort and protection for a week.  Pain is quite minimal and is well handed with pain medications and cool compresses.  There is some swelling and bruising around the eyes which clears up in 7-10 days.  The new shape is immediately noticeable, but complete healing after Rhinoplasty is gradual with swelling decreasing over 1-2 months.  Patients can expect to return to work or school within a week and strenuous activity within two to three weeks.

Botox


Small injections of BOTOX® around the eyes make the tiny wrinkles disappear, especially when squinting or laughing (thus, "laugh lines").  It is just as useful for the "frown lines" and forehead creases.  BOTOX® is injecting small doses of muscle relaxing medication to different areas of the face to temporarily prevent the function of overactive wrinkle-forming muscle tissues. This is possible by protein in BOTOX® which blocks the transmission of nerve impulses allowing the overactive muscles to relax.  The procedure takes only approximately 10 minutes.  Pain and discomfort are minimal.

Breast Lift

Do I need Breast Implants as well?

Will the scars fade?

In most instances fair-skinned patients fair much better as far as scars are concerned, while darker-skinned patients may form keloids or dark-colored scars.  Usually patients already are aware as to how they heal by their previous injuries or scratches.

What is the recovery like from Breast Lift surgery?

The recovery from Breast Lift Surgery is very much like from Breast Augmentation.  The pain is usually moderate, and with prescription pain medications patients do tolerate it very well.  Most patients can go back to their routine in less than a week and are back to work in about a week.  Mild exercise can resume in 10 days.  Swelling should subside in a couple of weeks, and Incision lines will continue to flatten and fade for up to 1 year.

Fine Lines & Wrinkles 
Fine lines and wrinkles are caused by the normal aging process, smoking, sun exposure and a number of other catalysts. As we age, skin loses elasticity and collagen thins, resulting in wrinkles and loose skin. New collagen production and antioxidant protection are essential in reducing skin wrinkling, as is protection from UVA/UVB exposure.

Skin care with healing properties and antioxidants should be incorporated into a daily regimen. Products with active growth peptides and micronutrients should be regularly applied to restore skin elasticity and stimulate collagen production. Additionally, you should always rely upon an effective sunscreen.

Acne Breakouts 
Breakouts occur when oil deep within small hair follicles or pores becomes waxy and clogs pores. To manage acne, first ensure your skin is clean and well-exfoliated. Second, eliminate bacteria which may inflame and infect pores. Products that effectively eradicate bacteria and exfoliate dead skin cells should be used. Benzoyl Peroxide, a popular ingredient in acne products, is only mildly effective at eradicating bacteria and is irritating in higher concentrations. Topical antibiotics lead to resistant strains of bacteria. Thus, antimicrobial cleansers that kill on contact are a preferred option.

Hyperpigmentation (Brown Spots)
Hyperpigmentation, commonly experienced as brown spots, is caused by UV light exposure which stimulates the melanin pigment production and forms brown spots. While there is no known cause, heredity appears to play a significant role. Other triggers include sun exposure, skin irritation, pregnancy, and certain medications, including birth control pills and hormone therapy. Once brown spots have appeared, avoiding triggers such as sun exposure and treating the skin with skin lighteners combined with accelerated exfoliation is effective.

Rosacea 
Rosacea (Acne Rosacea) is a condition characterized by red flushing, inflammation, small pimple breakouts, and in more severe cases a bulbous nose with thickened, oily skin. All cases are associated with early and persistent flushing in the face. Avoidance of triggers such as caffeine, sun exposure, alcohol and hot/spicy foods minimizes flushing and breakouts. Additionally, products with soothing properties will help to soothe irritated skin.

Dark Under-Eye Circles
The skin under the eye is delicate with few oil glands. It is thin and somewhat translucent, revealing capillaries beneath the surface which generate the appearance of dark circles. Factors such as heredity, illness, allergies, fatigue and aging contribute to their severity. As we age, dark circles become more apparent as collagen in the skin breaks down, resulting in thinner skin. While there is no nonsurgical, permanent solution, the appearance of dark circles can be managed by getting plenty of sleep, avoiding cigarettes, reducing alcohol and caffeine consumption, and taking special care of under-eye skin. Light, non-oily moisturizers that contain growth peptides (soluble keratin peptides) will effectively stimulate collagen production to restore skin thickness and elasticity.

Puffy Eyes
Puffy, swollen eyes may be triggered by a number of factors. Fluid retention, lack of sleep, allergies, sleeping flat on your back (without pillow elevation), and even heredity affect the degree of puffiness throughout life. As we age, eyes tend to appear puffier as skin around the eye begins to sag due to skin laxity and weakening tissue. Reducing salt intake, getting plenty of rest, drinking water and avoiding allergens can help manage puffiness. To care for the skin under the eye, use a moisturizing crème that will hydrate without clogging pores and will stimulate collagen production with growth peptides (soluble keratin peptides) to strengthen skin tissue.

Tummy Tuck/Abdominoplasty


A Tummy Tuck or an Abdominoplasty is a procedure that transforms a stretched out and bulging tummy to a firm and flat one.  Pregnancy causes the stomach to stretch from the inside, pushing out on the skin and muscle wall.  Significant weight gain and loss from pregnancies can result in loose and stretched skin that is not possible to reverse with diet and exercise.  Patients who had gastric bypass and banding surgeries also experience the same.  In both instances, even after much diet and exercise, patients are unable to have a firm and flat stomach.

How is a Tummy tuck performed?


The first step is to decide whether a Mini-Tummy Tuck or a Full Tummy Tuck is indicated.  This depends how loose the skin is, and how much to remove.  Dr. Diehl will discuss the options with you during your consultation.
A Mini-Tummy Tuck is performed when only the lower half of your abdomen is loose, and the belly button area looks youthful.  The Mini-Tummy Tuck is done by placing the incision very low, so that the scar is always under your panties or bathing suit, much like a cesarean scar.  The muscle is tightened from the belly button down to the pubis.  The incision is shorter than a full Tummy Tuck, the upper abdomen is left alone, and the belly button is not changed.  From the belly button down your tummy will be flat and the loose skin will be removed.
A Full Tummy Tuck is performed when both the upper and lower abdomen have been stretched, including the belly button.  More skin tightening is planned, and the abdominal wall is tightened from the very top of the abdomen down, past the belly button, down to the pubis.  Full-length tightening of the abdominal muscles from a Full Tummy Tuck results in the firm and flat tummy that is desired.  The skin is made maximally tight, giving you a flat stomach, and the belly button is reshaped to a youthful almond shape.  Most of the stretch marks will be removed as well.


What is the recovery for Tummy Tucks?


Mini-Tummy Tuck recovery is shorter and simpler than with a Full Tummy Tuck.  With Mini-Tummy Tuck, you will need about 10 days before returning to your normal routine. 
Full Tummy Tuck recovery is a bit longer.  The initial discomfort is handled with a “pain pump”, which is a continuous drip of anesthetic through a tiny tube.  Dr. Diehl will see you the day after your surgery to remove this, and to change your bandages for you.  A home-visiting nurse service is available should you choose; for the first few days the nurse will check on you and help with bandages and such.  The first 3 days feel like a cesarean section operation, and are handled with pain medications, such as Vicodin.  There are small drains which remain for about a week.   At about a week to 10 days you will feel well and want to start getting back to your usual routine.  However, a Full Tummy Tuck needs to heal a little while longer, and Dr. Diehl encourages his patients to take it easy for an additional week. 
Tummy Tuck patients, whether mini or full, are advised not to rush back to work; this is your time, that you deserve, and you should plan a leisurely recovery.  In summary, a tummy tuck patient needs two weeks off work, sometimes three, depending on the type of work.  Gym and sports activities may be resumed at about 4 weeks.

Facelift or a Rhytidectomy is a procedure where the loose skin around the face and neck is smoothed by removing the excess skin and the fat.  During a Facelift incisions are made in the hair line, temple region extending down in front of the ear and then around the back of the ear into the hairline.  The skin of the face and neck is then pulled upward producing a tightening and smoothing result.  The muscle layers underneath the skin are tightened as well during a Facelift procedure.

How long does the result from a Facelift last?

The results vary a bit from person-to-person as the face will continue to age as a natural process.  However, most people can expect their results to last between five and ten years.

What is the recovery like after Facelift surgery?

Most people are able to return to work between 10 and 14 days after their surgery.  Strenuous activities should be avoided for at least two weeks.
